1-on-1 support — built around their day, not ours.

In-Home & Community Support (sometimes called In-Home and Community Habilitation) is direct, personalized support designed to help adults with IDD live more independently in their own home and neighborhood.


Our DSPs work side-by-side with each participant on the skills that matter most to them: cooking, household routines, money management, hygiene, transportation, and social connection. The pace, the goals, and the methods are all shaped by the individual's ISP and preferences — never one-size-fits-all.

Daily Home Routines

Cooking, light cleaning, laundry, organizing — the practical rhythms of running a home, learned through doing alongside our DSPs.

Personal Care & Hygiene

Respectful support with bathing, grooming, and dressing — dignity first, always. Same-gender support available where preferred.

Safety & Health

Medication reminders, safety awareness at home and out, recognizing warning signs, accessing emergency services when needed.

Money & Errands

Budgeting, paying at the store, using a debit card, grocery shopping, navigating banking and bills with growing confidence.

Transportation

Learning to use public transit, riding along to appointments, building the confidence to navigate the community independently.

Social & Communication

Practicing conversation, building friendships, working through social situations with someone they trust by their side.

a day with IHCS

What a typical session might look like.

IHCS sessions follow the rhythm of the participant's day — at home for some, in the community for others. Below is a sample of how a session might flow for someone working on cooking and shopping skills as part of their ISP.

Every plan is unique. We follow your loved one's ISP and adapt continuously based on their progress and preferences.

1:00 PM
Morning routine support

DSP arrives, checks in, supports the participant through their morning routine with as much or as little prompting as they need.

11:30 AM
Plan & shop for dinner

Together, they plan a meal, build a shopping list, head to the store, and practice using the debit card at checkout.

1:00 PM
Cook & eat together

Back home — the participant leads the cooking, with DSP supporting and teaching kitchen safety along the way.

2:30 PM
Wrap-up & progress notes

Cleanup, a quick chat about what felt easier this time, and ISP progress documented before DSP heads out.

Who IHCS is for.

In-Home & Community Support is available to Pennsylvania residents enrolled in an ODP waiver, with services authorized by their Supports Coordinator.

Adults with intellectual or developmental disabilities

Including individuals with autism


Active referral from an SCO or AE

Supports Coordinator Organization or Administrative Entity


CPS authorized in the participant's ISP

Hours allocated under their waiver service plan

Three steps from referral to first session.

SCO referral

Your Supports Coordinator submits a referral with the individual's ISP and authorized IHCS hours.

Intake & matching

We meet the individual and family, learn their goals, and match them with a DSP who fits their personality and needs.

03

First session

Services begin. Our team communicates proactively with the SCO and family — no surprises, no gaps.
