In-home and Community Habilitation

Staff support and advocate for individuals with intellectual disabilities both at home and within the community. They facilitate rehabilitative and therapeutic activities, achieving Individual centered in-home and community goals. Additionally, staff assist with developing life and social skills relevant to daily living activities.

We’ll assist in integrating individuals into community experiences and activities, helping them connect with organizations, groups, and clubs to build social networks.   • Who qualifies for Alpha Health Services?

    Adults diagnosed with intellectual disabilities who have received Consolidated, Community Living, or PFDS waiver funding and a referral from a Supports Coordinator Organization (SCO) or Administrative Entities (AE) office.

  • How much does it cost?

    Our services are funded through Pennsylvania's ODP waiver programs — meaning there is no out-of-pocket cost for eligible individuals. If you're unsure whether you qualify, contact us and we'll help you figure it out.

  • Where do services take place?

    We provide support in your loved one's home and out in the community — wherever they need us across all 67 Pennsylvania counties.

  • What is a Direct Support Professional (DSP)?

    A DSP is a trained caregiver who provides 1:1 support to individuals with intellectual disabilities. They assist with daily tasks, companionship, skill-building, community access, and more.

  • How do I get started?

    Contact your Supports Coordinator Organization (SCO) or Administrative Entities (AE) office to request a referral to Alpha Health Services. Then call us at 267-361-6177 and we'll take it from there.

  • How long does it take to start services?

    Once we receive your referral, we'll reach out within 1–2 business days to begin the intake process and match your loved one with the right DSP.
